Please print and complete the roster form below. Prior to July 12, you may submit electronically to playballminster@gmail.com. Alternatively, you may submit a hard copy in to the nearest concession stand or your tournament director prior to your first game.
* Rosters MUST be submitted (in either manner above) prior to your first game. Failure to do so will result in forfeiture.
* Birth certificates will not be collected, but must be immediately available if requested by tournament directors.
For the tournament, we are also requiring a "Release from Liability" form be completed and signed for all participants (players and coaches). Please direct your parents to this form and submit all forms to the nearest concession stand prior to your first game.
